Fast, Reliable DOT Physicals
What to Expect During Your DOT Exam
Our DOT physical is a simple and hassle-free process that involves includes filling out all the necessary paperwork, taking your medical history, and performing physical exam.
Step One: Paperwork
The first step in the process is the DOT paperwork. When you leave the office, you’ll get a copy of forms along with the exam results to take with you when you get your CDL, and a copy to give to your employer. We’ll also keep a scanned copy of your exam at our offices. And after all the paperwork has been processed, you’ll get your very own DOT medical card to keep with you at all times when you’re driving.
Step Two: Medical History
To provide an idea of your general health and family background, you’ll be asked to fill out a brief one-page medical history detailing past illness, diseases that run in your family, and your overall health. We will review these questions together in order to assure there are no restrictions to your DOT card.
Step Three: The Exam
Once you’ve provided your medical history our team will review the information you’ve provided, go over any medical conditions you may have, and perform a basic urinalysis test. As part of your exam, our doctor will: take your blood pressure, test your range of motion, examine your eyes and ears and check your heart, lungs and abdomen.
Valid driver’s license and /or other photo ID.
Eye glasses or contact lenses, if applicable.
List of medications, and the name and contact information of the prescribing doctor.
Hearing aid, if applicable.
Medical release opinion letter (optional) from your primary doctor or specialist for any medical conditions, recent illness, injury, or surgery.
